Notice: If you would like to add a session to this program, please submit your session information using the form below by November 5. No sessions can be added to this program between November 5 and December 16 during the CFPC Cert+ closure. Participate in OCFP’s already certified program for your initial meeting:

Family doctors engaging in initial discussions regarding the development of integrated health care teams can earn credits through this existing certified program. This program ensures family doctors can claim certified credits for time spent in early interactive and collaborative sessions (1-credit-per-hour; 1.5 credits total). Because the OCFP has already certified this program, your team does not need to submit their own certification application.

Provide your program information:

Use the form below to provide us with the required program information. Please submit this information at least one week prior to your event. Our team will provide you with a certificate of attendance for your session once we’ve received this information and the required documents. A certificate is required for family physicians to earn Mainpro+ credits.
