Holding Risk with Care: Form 1 Decision‑Making in Family Medicine

When a patient discloses thoughts of self‑harm or harm to others, family physicians are often faced with one of the most complex and high‑stakes decisions in clinical practice: whether to complete a Form 1. In this session, our panelists will unpack the Ontario legal framework behind Form 1, what “reasonable grounds” and “serious bodily harm” actually look like in real office settings, and how to document and navigate these situations with confidence. Through case‑based discussion, we’ll explore how to have these conversations with skill, involve the right supports, and preserve, or even strengthen,  the physician-patient relationship. The session will also create space to acknowledge and address the emotional and professional toll these decisions take on family doctors.

The Practising Well Community of Practice has been certified by the College of Family Physicians of Canada and the Ontario Chapter for up to 1 Mainpro+Ⓡcredits. This Community of Practice includes a series of planned webinars. Each session is worth 1 Mainpro+Ⓡcredits, for up to a total of 12 credits.
