Certify your own session/program for your integrated health care team:
Family doctors’ involvement and leadership are key to the successful development of integrated health care systems.
This page contains information and templates to support your organization in applying for Mainpro+ certification. Below you will find a guide to help obtain Mainpro+ certification for your integrated care work and templates for the required documents that accompany your Mainpro+ application. Download the Mainpro+ Guide, along with application templates using the button below, and click through the documents for descriptions.
This will guide you through completing and submitting a Mainpro+ certification through the Canadian College of Family Physician’s (CFPC) Cert+ platform.
This is a sample document to list and describe the activities/events you are seeking certification for. It will also help you determine the total number of hours and credits for your program. You can use this document to keep track of when you disseminated certificates for your program/activity.
The fillable form is not a requirement, however, it is useful when drafting your certification application. This fillable form mimics what is required on the application. When you’re ready to submit your application, you can easily follow this document to complete the application questions on the CERT+ platform.
All members of your Scientific Planning Committee (SPC), speakers, moderators, facilitators and authors will be required to complete a COI (Conflict of Interest) form that includes information from the previous two years. You are required to upload your SPC COIs with your certification application. COIs for speakers can be kept on file.
All speakers and moderators of a Mainpro+ certified program are asked to disclose conflicts verbally and in writing to program participants using the CFPC 3-Step Disclosure Slide template at the beginning of a presentation. If slides are not used during the program, disclosures can be included in written program materials (e.g., conference program, course website, workbook, reading material) as applicable.
The agenda/schedule is a document to be uploaded in the Mainpro+ application outlining the duration of the program in order to match the requested number of credits. You should clearly indicate program time and interactive components on your agenda.
Needs assessment is the process of gathering information to identify the target audience’s learning needs. This document can be used to fulfill the needs assessment documentation requirement of your application.
Evaluation allows for the ongoing improvement of CPD programs and provides valuable feedback to program planners and faculty. Mainpro+ certified programs must include the following mandatory question in their program evaluations: “Did you perceive any degree of bias in any part of this program?” A checkbox beside the word “No” and a check box beside the word “Yes” should be provided for the participant completing the survey to select an answer. Additionally, the text, “If yes, please describe” with space allowing the participant to provide more information, should be added to obtain more details. You can use the evaluation form template as a starting point and add any questions that you wish to ask your participants.
The Mainpro+ application will request a copy of the planned communication/invitation for speakers/moderators and facilitators. The Scientific Planning Committee is required to communicate the following information within the invitation/communication: the program format, program learning objectives and Mainpro+ quality criteria: CMA (Canadian Medical Association) Guidelines for Physicians in Interactions With Industry and the Innovative Medicines Canada Code of Ethical Practices). Use this document (links to required information already included) and modify the details/learning objectives as necessary.
Participants are required to receive a certificate of attendance at the completion of the program in order to claim their Mainpro+ credits. Update this document with your organization’s logo, program name and session ID number.
The certificate of attendance must include the following:
Participant’s full name.
Program name (exactly as it appears in CERT+).
CERT+ session identification number.
Session date and location.
Number of credits for which the session is certified.
Type of credits.
Official Statement of Certification.
