SUMMARY:Clinical Application of the Long-Term Care (LTC) Fracture Prevention Recommendations for Frail Older Adults
DESCRIPTION:Long-term care (LTC) residents are more dependent\, frail and vulnerable\, and require more complex care than community-dwelling older adults. Yet many interventions intended to reduce fracture risk are not used consistently in LTC\, and clinical practice guidelines for osteoporosis have focused on the care of adults living in the community. This program aims to address existing care gaps by illustrating the application of pharmacological and non-pharmacological fracture prevention recommendations with frail older adults in LTC. After active engagement in this program\, participants will be better able to: \n\n\n\n\nAssess fracture risk using Fracture Risk Scale\n\n\n\nApply evidence-based recommendations for fracture prevention in LTC\n\n\n\nRecognize challenges and barriers to implement the recommendations and use enablers\n\n\n\n\nPresented by:\n\n\n\nAlexandra Papaioannou BScN\, MD\, MSc\, FRCP(C)\, FACP Professor of Medicine/Geriatric Medicine\, McMaster University Executive Director\, GERAS Centre\, Hamilton Health Sciences \n\n\n\nSidney Feldman\, MD\, CCFP\, FCFP  Chief\, Department of Family and Community Medicine\, Baycrest Health Sciences Associate Professor\, Family and Community Medicine\, University of Toronto \n\n\n\nDelivered in collaboration with the Ontario Osteoporosis Strategy. \n\n\n\nThere is no cost to attend. This one-credit-per-hour Group Learning program has been certified by the College of Family Physicians of Canada and the Ontario Chapter for up to 1 Mainpro+ credit.

SUMMARY:Supporting Children and Youth with Substance Use Concerns in Primary Care
DESCRIPTION:Substance use among children and adolescents is common\, and family physicians are often the first point of contact for youth and their families. This practical session will offer both clinical and lived experience perspectives for participants. During this session attendees will gain tools to identify problematic substance use\, learn how to navigate confidentiality and family involvement\, provide brief interventions and harm reduction strategies\, and determine when referral to specialized services is appropriate. \n\n\n\nThe Practising Well Community of Practice has been certified by the College of Family Physicians of Canada and the Ontario Chapter for up to 1 Mainpro+Ⓡcredits. This Community of Practice includes a series of planned webinars. Each session is worth 1 Mainpro+Ⓡcredits\, for up to a total of 12 credits. \n\n\n\nThis is the first of two sessions in a series focused on supporting children and youth at risk.
