SUMMARY:Caring for Adults with Intellectual and Developmental Disabilities
DESCRIPTION:Adults with intellectual and developmental disabilities (IDD) experience significant health inequities. Family physicians and primary care providers play a key role in addressing these gaps. In this session\, our panelists will review Ontario‑based research on health disparities and introduce the Canadian Consensus Guidelines on the Primary Care of Adults with IDD. We will explore practical approaches to physical and mental health care\, including preventive screening\, chronic disease management\, supported decision‑making\, and strategies for assessing behaviours that challenge. Practical tools from the Developmental Disabilities Primary Care Program will be highlighted\, with case examples demonstrating how they can be applied in everyday family practice. \n\n\n\nThe Health Equity Community of Practice for Ontario Family Physicians Community of Practice has been certified by the College of Family Physicians of Canada and the Ontario Chapter for up to 1 Mainpro+ credit. SUMMARY:Practical Strategies for Supporting Patients on OW & ODSP
DESCRIPTION:Family physicians regularly support patients navigating Ontario Works (OW) and the Ontario Disability Support Program (ODSP)—systems that can be complex and time‑consuming. \n\n\n\nThis session clarifies the key difference between OW and ODSP\, walks through ODSP application and medical review processes and demystifies additional income benefits. Designed for family physicians and primary care providers across Ontario\, this session focuses on actionable strategies to improve patient access to income and health benefits. \n\n\n\nBy the end of the session\, participants will be able to: \n\n\n\n\nDifferentiate OW and ODSP and identify when each program is most appropriate for a patient’s situation.\n\n\n\nComplete and support ODSP applications and medical reviews using clear\, eligibility‑aligned clinical documentation.\n\n\n\nIdentify\, explain and initiate SD\, MSN and DTC processes and connect patients to associated health benefits (physio\, dental\, vision).\n\n\n\n\nThe Health Equity Community of Practice for Ontario Family Physicians Community of Practice has been certified by the College of Family Physicians of Canada and the Ontario Chapter for up to 1 Mainpro+ credit. Primary Care Providers can play a key role in supporting patients to connect to income supports. In this session\, panelists will provide an overview of social assistance and income benefit programs. They will discuss strategies for completing successful Ontario Disability Support Program (ODSP) and Disability Tax Credit (DTC) applications and review the new Canada Disability Benefit.
