SUMMARY:Equity-oriented Primary Care Attachment
DESCRIPTION:In this session\, our panel will outline a pathway for equitable attachment to primary care. We will introduce a toolkit designed to support interdisciplinary teams—including those in Family Health Teams\, clinics and other primary care settings—in defining target populations and outcome measures\, building community partnerships and recruiting and retaining providers for new patients. Panelists will also explore strategies for optimizing team-based care and refining processes based on feedback from both patients and providers. \n\n\n\nLearn more about this CoP here: https://ontariofamilyphysicians.ca/supports-for-family-doctors/health-equity-cop/ \n\n\n\nPanelists:\n\n\n\n\nDr. Curtis Handford\, Family Physician SMH DFCM\n\n\n\nDr. Noor Ramji\, Family Physician\, SMH DFCM\n\n\n\nNassim Vahidi-Williams\, Manager\, Patient and Community Engagement\, SMH FHT\n\n\n\nChantal Sorhaindo\, Nurse Practitioner\, SMH FHT\n\n\n\nLauren Brouhard-Chuck\, Social Worker\, SMH FHT\n\n\n\n\nModerator:\n\n\n\n\nDr. Jobin Varughese\, OCFP President\, Family Physician\, Interim Assistant Dean of Primary Care Education for the School of Medicine at Toronto Metropolitan University (TMU)\, Brampton\, ON\n\n\n\n\nOur Partners \n\n\n\nThe Ontario College of Family Physicians has partnered with the Department of Family and Community Medicine to deliver this community of practice.  \n\n\n\nThis Community of Practice has been certified by the College of Family Physicians of Canada and the Ontario Chapter for Mainpro+Ⓡcredits. \n\n\n\nDownload the Resources:\n\n\n\nSession slides (PDF)Resources (PDF) \n\n\n\nWatch the session\n\n\n\n\n\n\n\n\n\nListen to the Audio\n\n\n\n\n\n\n\nSelf-Learning Program:\n\n\n\nThis session is part of the Health Equity Self Learning Program.
